• VA claim information and C&P exam results
• Information shared during forensic telehealth evaluations

• Providing forensic psychological evaluation services and preparing Independent Medical Opinions (IMOs), Independent Medical Evaluations (IMEs), and Nexus Letters

• Complying with legal and regulatory obligations

4. How We Share Your Information

• With your written consent: When you authorize sharing with VSOs, accredited attorneys, claims agents, or other representatives
• HIPAA Business Associates: Vendors with signed Business Associate Agreements (BAAs) — Zoho (Forms, CRM, Mail, Books), Doxy.me, and Twilio
• Legal requirements: When required by federal or state law, court order, or valid government process
• Emergency situations: To avert serious and imminent harm to you or identifiable third parties

5. No Guarantee of VA Outcomes

Nothing in our reports, communications, or services constitutes a guarantee that your VA disability claim will be approved or rated at any particular percentage. All clinical opinions are independent medical opinions based on available records and clinical interview only. Client is solely responsible for how any report is used, submitted, or interpreted in any legal or administrative proceeding.

6. Your HIPAA Rights

• Right to Access: Inspect and receive a copy of your PHI within 30 days of written request
• Right to Amend: Request corrections to inaccurate or incomplete PHI
• Right to Restriction: Request limits on use or disclosure of your PHI
• Right to Accounting: Request a list of disclosures made of your PHI in the prior six years
• Right to Complain: File a complaint with FarmUS or the U.S. Dept. of Health & Human Services at hhs.gov/ocr — without retaliation

7. Records Retention

PHI and evaluation records are retained for a minimum of seven (7) years from the date of service, or as required by applicable law and professional standards. Records are securely destroyed upon expiration of the retention period.

8. Data Security

We implement 256-bit SSL/TLS encryption, HIPAA-compliant platforms with signed BAAs, role-based access controls, and regular security assessments to protect your information.

9. Changes to This Policy

We reserve the right to update this Privacy Policy at any time. The revised policy will be posted with an updated effective date. Continued use of our services constitutes acceptance of the updated policy.
