Telehealth Informed Consent

1. Provider Information

Medical Provider: Dr. Rebekah Harris-Creamer, Ph.D., Licensed Clinical Psychologist
PSYPACT® Participant — Authorized to provide interjurisdictional telehealth services in participating states and jurisdictions only. Services are provided solely in jurisdictions where the provider is legally authorized to practice at the time of service.

All clinical services are performed solely by Dr. Rebekah Harris-Creamer, Ph.D., Licensed Clinical Psychologist. No other individual performs, supervises, or directs clinical services.

2. Nature and Scope of Services — Forensic/Administrative Only

FarmUS Health Services LLC provides the following services exclusively via telehealth:

• Independent Medical Evaluations (IMEs) for VA disability claims documentation
• Independent Medical Opinions (IMOs) and Nexus Letters based on available records and clinical interview
• Medical records review and administrative eligibility assessments

No guarantee is made that any report, opinion, or letter will result in approval of a VA claim or any specific disability rating. All opinions are independent medical opinions based solely on available records and clinical interview.

All telehealth evaluations are conducted via Doxy.me, a HIPAA-compliant video conferencing platform. No software download is required.

3. Voluntary Participation

Participation is entirely voluntary. You may withdraw consent at any time by written notice to support@farmushealthservices.com. Withdrawal does not entitle you to a refund for services already rendered.

4. Potential Risks of Telehealth

• Technology failures may disrupt sessions
• Despite HIPAA-compliant platforms, no electronic transmission is 100% guaranteed secure
• Telehealth is not appropriate for all clinical presentations or psychiatric emergencies
• In a mental health emergency, the provider may have limited ability to provide direct in-person assistance

6. Client Responsibility for Use of Reports

Client is solely responsible for how any report, opinion, Nexus Letter, or other document produced by FarmUS is used, submitted, shared, or interpreted in any legal, administrative, or VA proceeding. FarmUS bears no liability for outcomes resulting from the use, submission, or misuse of any document by the client, their attorney, VSO, or any third party.

7. Confidentiality and Exceptions

All information shared during evaluations is confidential and protected under HIPAA. Mandatory exceptions include imminent risk of harm to yourself or others, suspected abuse or neglect, court-ordered disclosure, and your written authorization to share information.

8. Technology Requirements

• Device with camera and microphone (computer, tablet, or smartphone)
• Stable internet connection
• Private, quiet location for the evaluation
• Compatible browser: Chrome, Safari, Firefox, or Edge

9. Consent Acknowledgment

By using our services, you confirm that you are at least 18 years of age, have read and understood this Telehealth Informed Consent, voluntarily agree to receive forensic/administrative evaluation services via telehealth, and understand that no guarantee of any VA outcome is made.
